COACH ADDRESS IS NOT NEEDED FOR THE SPRING SEASON
You must submit a separate form for each age division (age group) you are declaring.
The scheduler will send an email to the address that is used in the Rep Email field confirming he received the declaration. If you do not receive a confirmation email within two days, email or call the scheduler using his contact info on the league website. - Do Not assume that the scheduler has received your declaration form. - It is your responsibility to confirm that your declaration has been received.
TIME SAVING TIPS
You can copy data from another source such as a Word document or an email & paste it into the appropriate form fields. This will save you time if you already have the data on file or if you need to submit multiple forms.
If you are submitting multiple declarations you can use the browsers back arrow to return to this page after you submit the form and all the information will still on the form. This is a time saver also but just for the Rep info area if the rep info is the same... HOWEVER... Please make sure to change the age division and any other info that is different like the coach info, field info and game start times etc.
